SL1 Accident (1961)
On January 3 1961 SL1 reactor was being prepared for a restart following a maintenance outage. The control rods were being withdrawn to reconnect them to the drive mechanism, when suddenly one rod was removed to far, causing the reactor to become prompt-critical. The resulting power surge led to an explosive vaporization of the water, which in turn created a water hammer, ejecting the control rods and the shield plugs.
