Live-Related & Deceased-Donor Kidney Transplantation | Dr. Ravikumar NR | Apollo Hospitals

"I would like to speak on renal transplantation, which we also call kidney transplantation. This is of two forms: live-related kidney transplantation and deceased organ donor transplantation. Both have a lot of advantages.
In general, when the patient is young—say, a recipient in the age group of 18 to 60 or 65—renal transplantation is the best option for patients with end-stage kidney disease, what we call ESKD. We need donors for this, and we follow general rules of blood transfusion. Blood group compatibility is very important, and we perform several tests to ensure both donor and recipient are safe. We do not take any chances with the donor.
Generally, the outcomes of live-related kidney transplantation are very good. One-year graft survival and one-year patient survival are above 95%, and five-year graft and patient survival rates are also in the 90s. So, when done meticulously, the outcomes are excellent. Donor factors also play a key role. We must choose the donor carefully—ideally, they should be related, and there must be proper blood group compatibility. In addition, several other factors are involved, though that goes beyond the scope of this talk. It is a very careful and detailed process.
Deceased organ donor transplantation is another excellent option, mainly for patients who do not have live-related donors. This too is suitable, and again, we evaluate various donor-related factors. One of the most important is ischemia time. My thesis was actually on variables in deceased organ donor transplantation. The sooner the harvested kidney is engrafted, the better the outcome.
In this type of transplant too, we look at blood group compatibility and CDC crossmatch. The outcomes are slightly better than dialysis and have 90 to 95% one-year graft and patient survival. So this is equally effective. We are very pleased to see that many of our patients do well on the deceased donor organ list as well. It’s an important modality."
