CVR - Aeroflot Nord Flight 821

On September 14th, 2008, a Boeing 737 of Aeroflot-Nord (owned by Aeroflot) operated a flight from Moscow to Perm. There were 88 people on board. The flight crew consisted of:
• Captain: Rodion Mikhailovich Medvedev
• First Officer: Rustem Rafailovich Allaberdin
During the climb to flight level 310, the captain drank a large amount of alcohol (how exactly this happened is not specified), as a result of which he was unable to fly the aircraft normally.
The air traffic controller announced the "MH4B" approach plan, which was longer than the xMH4A" that the pilots set into the flight plan. The first officer due to poor preparation and bad skills entered the new approach plan incorrectly, which caused more difficulty for the drunk captain.
The captain forced the first officer to make an approach, while he himself negotiated with the controller in a clearly intoxicated state. The controllers request to go around was ignored three times, after which there was even a question whether everything was okay in the crew.
Soon, the first officer asked the captain to take control, to which captain replied "I can't either!" However, for unknown reasons, he turned the yoke to the left roll of 76°
The aircraft rolled over making a barrel roll and crashed into a railway line right in the center of the city at a speed of 250 knots
Nobody on the ground was hurt. All 88 people on board died.
Final Report:
• An autopsy showed that the captain's blood contained a high level of alcohol of 0.05% to 0.11% abv.
• The captain became spatially disoriented and flipped the aircraft.
• The aircraft had been flown with a throttle problem increasing the workload on the pilots.
• Poor crew resource management.
• Both crew members submitted falsified documents to the airline showing they had passed preflight courses.
How the captain was able to consume alcohol on board remains unknown. After this crash, "Aeroflot Nord" was dissolved.
