Why is Botox not always the answer?
Botox is an excellent starting point for eye rejuvenation- especially in patients between the mid 20s to the early 40s. This can prevent crows feet and also reduce eye bags due to excessive muscle contraction.
Once you hit the late 40s and 50s, Botox to the eye area should be combined with other treatments. Why? It is because you skin, especially under the eye gets looser as we loose collagen and the supporting structures. Botox only to the area that supports loose skin can make the wrinkles under you eyes look worse. Combining Botox with Blepharoplasty either by surgery or with laser is the ideal method of creating harmony- that is addressing skin texture, tone and volume.

What can be done for prominent eye bags?
Patients in the early 20s to early 30s usually have prominent eye bags due to strong muscles around the eye area. This can very easily be rectified by muscle relaxants such as Botox to the area. This releases the tension of the muscles improving eye bags instantly.

Patients in their 30s-40s may have a combination of the above, but most importantly mid volume fat loss. This is universally seen in just about everyone in this age group. This makes the fat pad more visible – because you have loss volume surrounding the pad itself. Easily fixed with a tiny bit of dermal filler.

Patients who are in their late 50s to 60s may have prolapsed bags, and may require lower eyelid Blepharoplasty or surgical excision of loose skin followed by diathermy or excision of the fat pad.

Why are combination treatments best for rejuvenating the eye area?
In some cases eye rejuvenation is straight forward- 0.5ml of filler in the cheeks if you have subtle volume loss. Possibly Botox to the crows- if you are in the early to late 30s. As one ages, changes occur. You will loose more mid facial fat, and your orbital rim recesses – hence hollowness will be more pronounced. Additionally the skin will be more lax/ loose and permanent wrinkles will occur. This is when combination therapies, including surgical blephroplasty, laser eyelid surgery and fillers come in to play. Each aspect of treatment is designed to improve skin texture, tone and volume. This gives a natural but beautiful look to the eyes and surrounding areas.

Can dark circles be treated with laser or filler?
Treatments will depend on the cause of dark circles. Dark circles due to skin laxity can be treated with fillers, or with multiple sessions of Q Switch lasers. CO2 and Erbium Fractional lasers can also tighten skin.
Why do I think eye rejuvenation is high on the priority for facial rejuvenation.

What is the first thing you notice when first meeting someone? Their eyes. That says it all. In my opinion the eye area is one of the most important aspects of facial rejuvenation, combined with improved skin texture and skin tone, this provides the framework for facial harmony. I find the eye area extremely challenging, but if performed correctly, the changes can be either subtle and natural, or dramatic, as with eyelid surgery/ blepharoplasty and laser. Every patient is different, some present with eye bags due to loss volume -common in the late 30s and early 40s, some with strong eye muscles that present as bags. Easily treated with Botox. Some patients have droopy upper eye lids, some with loose skin under the eyes. Some patients present with the whole lot. Sometimes a brow lift is need, other times a mini S face lift. I can not emphasise the necessity for facial harmony, namely blending the eye area with the rest of the face. In my opinion, Botox to the crows feet only play a very, very small role in eye rejuvenation.
