The medial malleolus is the bony bump on the inside of the ankle, formed by the lower end of the tibia. It anchors the deltoid ligament, one of the main stabilizers of the ankle joint, so a fracture here is about more than a broken bone. It can affect how well the joint holds together under load. #brokenankle

When I evaluate a medial malleolus fracture, two questions guide the plan. Is the ankle stable, and how far has the fragment shifted? I rely on weightbearing or stress radiographs to answer the first, and I measure displacement carefully on imaging. That distinction changes everything that follows.

For fractures with minimal displacement and a stable mortise, nonoperative care often works well. That usually means a short period in a cast or walking boot, protected weightbearing, and close follow-up with repeat films to confirm the bone holds its position. Many of these patients heal fully and return to their prior activity.

When the fragment is displaced or the joint is unstable, surgery restores alignment and protects the joint surface. I typically use one or two screws, and for vertical shear patterns an antiglide plate adds stability. The goal is always the same. Anatomic reduction and a congruent joint, because even small shifts in the talus change how pressure is distributed across the cartilage.

What I tell patients most often is that recovery is a process. Bone healing takes weeks, but full functional recovery can take longer, and rehabilitation drives the result.
