NHA Hosing Program Pre-Qualification Requirements for OFW's || Dad's InfoTV

list of requirements to avail our Government Employees Housing Program.
To apply in the said housing program, you must submit the following for Pre-qualification as listed below:

1. Duly accomplished Application Form (Notarized)
2. Proof of Income. Any of the following:
a. Employer’s Certificate of Compensation
b. BIR Certified Income Tax Return
3. Proof of Identity/Civil Status
a. For Single Applicants:
Birth Certificate
b. For Married Applicants:
Marriage Contract
c. For Separated Applicants:
Affidavit of Separation-in-Fact (for applicants not legally
separated/annulled)
4. Photocopies of two (2) government-issued ID with 3 Specimen
Signatures
5. Folder (any color)
