RSA Makes Sense. Not Heresy. Part Two

Overcome the fear or lack of practice of doing RSA.
Training is hardest for those with a degree and licenses to work in the field. Unless a mandate is enforced or new guidelines come from AHA or ILCOR, you aren't forced to change management of emergency airways.
It is best to supraglottic device that can allow decompression of gastric gases and juices and contents. But remember I am giving RSI drugs to enable the patient to tolerate this SGA to get the o2 sats up. Hopefully after the pulse ox lag of 45 seconds to a 1 minute passes. Use end tidal co2 to prove that you have a good seal/adequate deep enough placement of the SGA. Prove that you are able to ventilate through the SGA and you assume you are oxygenating as well.
Think of RSA as preoxygenation with RSI drugs.
I don't know if I can ventilate this patient once paralyzed with Bag Valve Mask ventilation. I really don't care since I am placing an advanced airway. With the plan the sats are up as high as they can for at least 2 minutes I hope, I assume I have preoxygenated the patient enough to allow me to remove the SGA and place it on their chest within my arm's reach and the intubate with my best First Pass Success Laryngoscope device. Or if it was an intubating SGA that do exist, I use my Hopefully my disposable Fiberoptic scope equivalent and start searching for Carina.

Who here has tried RSA?
In a way, you all have if you inserted an SGA when intubation failed after RSI drugs had been given, and the patient desaturated on pulse oximetry. The only thing is I am buying a safer environment for intubation by preoxygenation with SGA and not pursuing BVM preoxygenation. Head to head SGA is more successful in providing preoxygenation and rescue oxygenation.

Perfect scenario in the Airway911 video, stemi on ekg, hypoxia from acute pulmonary edema. This patient may decompensate on lying flat on the cathe lab table even with a bipap/NIV mask on. Preemptive intubation may be desired but sats barely 90% or high 80s% while on the NIV.
