Rare Operating Room Footage: Pectoralis Major Repair with Sports Medicine Surgeon Dr. Nimit Patel
Fellowship-Trained Sports Medicine Surgeon, Dr. Nimit Patel, recently took us inside the operating room to get a rare look at an uncommon surgery: Repairing a Pectoralis Major Complete tear.
The pec major is a powerful muscle originating from the sternum and clavicle with its tendon attached to the humerus near the biceps tendon (insertion point: Crest of the greater tubercle of the humerus).
The pec major and pec minor make up what we refer to as our chest muscles. Our chest muscles are essential for upper body strength (throwing, pushing), good posture, and even breathing.
This injury is so uncommon due to the amount of force typically required to separate tendon from bone. Very few pec injuries result in a complete tear as is the case with Dr. Patel's patient here. Instead, many pectoralis injuries are typically partial tears or ruptures within the muscles that can be treated with non-surgical management and a carefully structured physical therapy program.
Those most at risk for this type of injury are generally bodybuilders and weightlifters, however as individuals continue weightlifting later in life, we are seeing increasing numbers of pec tears in those 50 and older.
Due to the dramatic nature of this injury, the sudden weakness accompanied by a series of pops and a noticeable deformity, patients typically seek medical attention quickly.
Symptoms you might have with a pec tear include:
- Bruising in the arm and chest
- Pain in the upper arm and chest
- Deformity of the upper arm and chest
- Weakness or loss of strength
- Swelling around the affected muscle
- Dimpling or pocket formation around the armpit
If this injury isn't addressed very soon after it occurs, the tendon can 'heal' by adhering incorrectly to different internal structures instead of in its proper place. This can make the surgical procedure intricate and complex, requiring the surgeon to locate the out-of-place pec tendon, detach it, and carefully reattach it correctly to the humerus.
As you can see here, even though the patient and Dr. Patel were able to schedule surgery for only 5 weeks after his injury, Patel and his team bring us through this exact process to give his patient the best chance of recovering and getting back to full function.
