In this clip, Prof Philipp Sommer highlights the risk of ‘mal-rotation’ during pulsed field ablation.

A fluoroscopy-only approach to catheter rotation can be misleading. In up to 50% of cases, rotation may appear correct, yet result in positioning one spline away from the intended overlap. The consequence is incomplete lesion coverage around the pulmonary vein circumference.

Using catheter markers to guide precise rotational alignment enables deliberate overlap with prior lesions and supports more durable, contiguous ablation lines.
