Landmark-Guided Midline Spinal Anesthesia (Step-by-Step Technique)

This illustrates how I personally perform surface-landmark-guided midline spinal anesthesia in the sitting position.
The main points to highlight and watch for, which are critical when faced with a "difficult" spine and small narrow spaces, are:
- finger and hand positions (practice until it is second nature).
- how to control the overlying skin (fix it, make parallel shifts, over underlying bony spine).
- the small, subtle and controlled incremental changes in trajectory when making redirections
- acquiring a tactile sense ("feel") of the different tissues that the needle tip passes through.
- developing a relaxed yet controlled grip and handling of the needle and introducer, ensuring that they travel in a straight line without applying any torque that might cause them to deviate from the intended trajectory.
