Implementing a Nurse Navigator Program Within the Emergency Department
for the Unhoused

This Doctor of Nursing Practice project addressed high emergency department (ED) utilization among unhoused patients by implementing a nurse navigator program to improve care coordination and follow-up access. Through staff education and collaboration with the local Health Department, the initiative reduced readmissions and improved ED throughput. The project demonstrates how nurse-led, community-partnered interventions can enhance care for vulnerable populations while improving system efficiency.
