Alexandria Orthodontics Showcase: Overbite Correction with Extractions and Retention

Our case of the week follows a patient whose initial concern was his large overbite and his flared teeth. His orthodontic diagnosis included a 10mm overjet and a 100% deep overbite with palatal contact. His molars contacted in a full cusp class II, meaning that his upper teeth were shifted a full tooth forward on both sides. He also had profile concerns. Because of the flared upper teeth and overjet, soft tissue concerns included a protrusive upper lip and a protrusive lower lip with a deep mentolabial fold due to the downward guidance from the flared upper incisors into the lower lip. ⁠

Treatment included braces for 20 months with the extraction of the first premolars and then the retraction of the upper anterior 6 front teeth. At the conclusion of his treatment, he decided against the fixed lingual retainers. His case was followed beyond the normal 12 month retention checks into 30 months. Comparing changes from the records taken on the day of debond (removal of braces/retainer delivery), we observed the following: stable space closure in the areas of extraction, settling of the canines and posterior teeth into contact, and good soft tissue aesthetics through growth and development. There was some mild relapse of the anterior deep bite though it did not translate into lower crowding.⁠

The patient was extremely happy with his smile and still misses being in treatment. It is important as orthodontic clinicians that we stress the importance of lifelong retention. Often, the retentive phase is most difficult than the treatment phase since the patient will be responsible for keeping up with the retention protocol. We should continue to reinforce and reward patients for continuing their efforts in maintaining their amazing smiles into retention.⁠
