Treatment of Erectile Dysfunction -Pharmacology

Drugs licensed for the treatment of erectile dysfunction include
1.Phosphodiesterase type 5 (PDE-5) inhibitors
2.VASODILATORS
3.ANDROGENS
They have revolutionized the treatment of erectile dysfunction.
Caution
Caution is needed in patients with
cardiovascular disease,
anatomical deformation of the penis, e.g. Peyronie’s disease,
those with a predisposition to prolonged erection,e.g. in sickle-cell disease.
They are contraindicated in patients who are on
nitrates and
in patients with a previous history of non-arteritic anterior ischaemic optic neuropathy.

SILDENAFIL
INDICATION -Erectile dysfunction and Pulmonary hypertension
MODE OF ACTION
Sildenafil is a phosphodiestearase type 5 inhibitor that works by improving blood flow to the penis.
During sexual stimulation ,penile nerve endings and endothelial cells release Nitric oxide.
This activates enzyme guanylate cyclase resulting the production of cGMP –A Vasodilator

SIDE EFFECTS
Headache ,flushin,dyspepsia,priapism and allergic reactions

DOSAGE
25mg ,50mg or 100mg(max dose) once daily 1-4 hours before

TADALAFIL
Works same as sildenafil
PRECAUTIONS
Cardiovascular disease, active peptic ulcer, hepatic or renal impairment
DOSE
50mg initially as a single dose 1 hour before sexual activity
Its effect can last for 24 hours
