STANDARD ITEMS NEEDED FOR NACA
Employment History: complete at least the past two years
Landlord History: complete at least the past two years
Bank Information: complete all bank account information

UPLOAD TO WEBFILE
Paystub: upload most recent 30-days for W2 earners
W-2: upload most recent two years for all employers
Bank Statements: upload most recent 90-days for all accounts (12 months if self-employed
Identifying Business Deposits - BD and Business Expenses - BE)
1099 Forms (only for self-employed): upload most recent two years
Tax Returns: upload most recent two years for all Members
Other income documents that apply: upload (SSI, disability, pension, etc.)
Child Support & Alimony: upload divorce decree, or court order and evidence of receipt for past 12-
months
Credit card: upload most recent statement(s) for all accounts
Student Loan: upload servicer statements documenting current payment plan
Budget: upload three-page budget from the Qualification Workbook
Initial Assessment: upload Member's Initial Assessment from the Qualification Workbook
Participation: upload the Participation Commitment & Skills Directory from the Qualification
Workbook

PAY
Membership Dues: Pay annual Membership fee due every January 1st
Credit Report Fee: pay the credit report fee in advance ($13.08 for single and $13.41 for joint) Counseling Session Requirements.
