UPDATE: If you are travelling from the EU there are no longer any restrictions. Therefore this form is not required.
UPDATE: If you hold an EU Digital covid certificate, or EU equivalent of vaccination against covid-19 or a negative test, or certificate of recovery you are no longer required to complete this form (This includes the NHS covid pass). If you do not hold an EU certificate or EU equivalent, you will still be required to complete this form.

Hello everyone, I hope this video can help you to complete the Spanish health form which is required to be completed by all those travelling to Spain, no matter what country you are travelling from. It is filled out from a UK perspective but is similar for all countries.
