A rhinoplasty, commonly referred to as a “nose job,” is a surgical procedure that reshapes the nose. It is one of the most commonly requested plastic surgery procedures and something that Dr. Caridi performs routinely at Westlake Plastic Surgery in Austin, Texas. This surgical footage video is an educational presentation of the components of a classic rhinoplasty / nose job including a pre-operation anatomic analysis well as a detailed presentation of the surgical procedures, concluding with a post-operative exam during which the dorsal splint is removed.

Dr. Caridi starts be describing the anatomy of the nose. The rhinoplasty patient does not display significant alar flaring, or excessive curvature of the curved portion of the lower nose. He and the patient are targeting a more feminine shape with a slightly higher angle around the columella as well as reduced fullness of the nasal ridge, also called dorsal ridge or dorsum, which is higher and projects out more than the patient would like. Dr. Caridi is going to fracture the bone to reduce the dorsal projection and also slightly narrow the area to the sides of the nasal ridge. To achieve this, Dr. Caridi will fracture the nasal bone under the nasal ridge and place a small incision at the base of the columella between the nasal vestibules.

During the surgical procedure, Dr. Caridi marks the stair stepped incision he will make on the columella and notes the patient's deviated crooked septum. He notes the position of her nasal bone and her lateral nasal cartilage including the upper lateral cartilage and lower lateral cartilage. He marks the area on the nasal bone to reduce the roof of the nose and marks the nasal tip points that he will use to raise up the tip of the nose.

The surgeon then describes what do expect for a rhinoplasty recovery. The procedure take a few hours in the operating room under general anesthesia. The days following a nose job involve recovering from the anesthesia. A splint will be applied and Telfa surgical pads are inserted for the first days. Breathing is obstructed during the days immediately after a rhinoplasty due to the swelling. Stitches and the splint come off in about a week.

The surgical footage video continues on to "unroofing" the nose - separating the cartilage and tip of the nose from the nasal bone under the nasal ridge / dorsal ridge / dorsum. The surgeon shows the layers of the septum, upper lateral cartilage and the nasal bone. The surgeon reduces the projection, or hump, of the nasal ridge using an incremental approach. The bone needs to be reduced with a rasp that chisels the bone. Dr. Caridi starts with a conservative approach and incrementally reduces the nasal ridge to a more feminine appearance. The results are immediately visible with an ideal harmonious proportion between the proximal portion and the distal portion.

Dr. Caridi continues to shape the nose being careful to avoid the "Michael Jackson look" associated with bad nose jobs. He begins "closing the roof" with stitches after several rounds of analyzing the uniformity and evenness of the shape, changing from a convex to a concave appearance of the cartilage. He then adds local anesthetic and performs a percutaneous osteotomy - breaking the nose bone to narrow the appearance of the base of the nose. The bone cracks and the surgeon holds the bone in place to control swelling.

The final step of the rhinoplasty involves closing the nose and analyzing the final appearance - the tip of the nose, the even dorsal bridge, and the more ideal feminine appearance the patient desires. Telfa surgical pads are inserted and skin glue is applied to hold the dorsal splint in place then a tip dressing is added to give the tip of the nose additional support. Final shaping the nose is achieved during post operative healing.

One week after the procedure, the patient returns to the cosmetic surgery office for a post-operative exam. The splint is removed, swelling is checked and the shaping of the nose is analyzed. The healing continues for several months - the patient sends in selfie photos showing how the nose has healed several months after the procedure.
