Runway Hero's Formstack ICD-10 Lookup Module Demo

The ICD-10 module allows for a dynamically searchable field which allows users to find appropriate ICD-10-CM codes. ICD-10-CM (International Classification of Diseases, 10th Revision, Clinical Modification) is a medical coding system for classifying diagnoses and reasons for visits in U.S. health care settings. The lookup is based on the official database from the CDC's National Center for Health Statistics (NCHS) and is always up to date. This can be used on a variety of forms in healthcare and insurance settings.
