On the 25th of February 2009 Turkish Airways flight 1951, a Boeing 737-800, from crashed 0.9nm short of runway 18R in Amsterdam, Schiphol international airport.
The aircraft was flown with 3 pilots in the cockpit, none of them noticed how the airspeed became dangerously low during the approach or how the Auto-throttle was working against them.
