"Central Venous Access Device: Dressing Change - Before beginning this procedure always perform the following steps to ensure patient safety. Perform hand hygiene, provide patient privacy, introduce yourself, use to patient identifiers, verify allergy status, perform environmental safety check, ensure proper body mechanics, gather all supplies, provide patient education. Materials needed for
CVAD dressing change: mask, dressing change kit, transparent dressing, bio-patch, chloraprep, skin prep, sterile gloves. Assemble supplies before entering patients room. The Procedure: Don clean gloves and mask, ask patient to turn head away from insertion site, provide patient with a mask, inspect and palpate side around dressing for swelling or tenderness, remove dressing by pulling it towards catheters insertion site, remove antimicrobial patch, measure external portion of catheter, compare previous length to what was previously documented to detect migration, discard
dressing and gloves, perform hand hygiene, open sterile dressing kit and dawn sterile gloves, cleanse insertion site, first must physically cleanse with alcohol in a circular motion for at least 30 seconds, be cautious not to tug the line and cause the line to dislodge, do not cross sterile field, then must
physically cleanse can with chg swab under central line in a back-and-forth motion for 30 seconds,
must physically cleanse the central line with a new CH G swab from insertion site upwards, allow to air dry, apply skin prep apply antimicrobial patch (bio patch), apply transparent dressing, coil external
portion of catheter and tape in place, label new dressing with date, time and initials. Dispose of soiled supplies in biohazard bag, return bed to appropriate height, brakes locked, appropriate side rails up and call light within reach. Dispose of trash and proper receptacle, wash hands / CDC guidelines."
