✈️ Summary of the Air India Flight VT-ANB Accident – Preliminary Report (12 June 2025)

PDF = https://aaib.gov.in/Reports/2025/Accident/Preliminary%20Report%20VT-ANB.pdf
Accident Overview:
Aircraft: Boeing 787-8 (Registration: VT-ANB)

Flight: AI171 (Ahmedabad to London Gatwick)

Date/Time: 12 June 2025, 13:39 IST (08:09 UTC)

Location: Ahmedabad, India – shortly after takeoff

Fatalities: 12 crew + 229 passengers + 19 people on ground = 260 fatalities

Survivors: 1 passenger survived (serious injury)

Phase of flight: Initial Climb

Crash Site: The aircraft hit buildings ~0.9 NM from end of runway 23, near BJ Medical College hostel.

🔍 What Happened? (Timeline Summary)
Pre-flight Checks:

Aircraft had arrived earlier from Delhi as AI423.

A minor defect ("STAB POS XDCR") was noted and cleared before the flight.

MEL (Minimum Equipment List) items active, but within valid limits.

Flight crew were well-rested and passed alcohol checks.

Aircraft was within weight limits and weather was clear.

Takeoff (08:07 UTC):

Aircraft took off from Runway 23.

Takeoff speeds were achieved normally:

V1 at 153 knots

VR at 155 knots

Liftoff at 08:08:39 UTC

Sudden Engine Shutdown (~08:08:42 UTC):

Just after takeoff, both engine fuel cutoff switches transitioned from RUN to CUTOFF.

Resulted in both engines shutting down mid-air.

Cockpit voice recorder captured one pilot asking:

“Why did you cutoff?”
The other replied:
“I didn’t do it.”

RAT Deployed + Attempted Recovery:

Ram Air Turbine (RAT) deployed automatically due to power loss.

Pilots tried to restart both engines by returning switches to RUN.

Engine 1 began to recover; Engine 2 failed to stabilize.

Aircraft lost altitude rapidly before engines fully recovered.

Crash (~08:09:05 UTC):

A MAYDAY call was made.

Aircraft struck trees and then several buildings.

Broke into pieces and caught fire.

Emergency response began at 08:14:44 UTC.

📌 Key Technical Notes:
Cause (Preliminary Indication):

Uncommanded engine shutdown via fuel cutoff switches — why they flipped remains under investigation.

Similar fuel control switch design had advisory bulletin (SAIB NM-18-33) in 2018, but not mandatory.

No known recent defect related to these switches on this aircraft.

Black Box Data:

Two flight recorders recovered.

Forward recorder had usable data (captured full event + audio).

Aft recorder was too damaged to extract data.

Data confirmed: fuel switches were toggled off mid-air unexpectedly.

Damage:

Aircraft completely destroyed.

Impacted 5 buildings, with structural and fire damage.

Debris spread over ~1000 ft x 400 ft area.

📋 Investigation Progress:
On-site work completed, engines and major parts quarantined.

Fuel samples from refueling trucks were clean.

Detailed analysis of crew audio, engineering data, and postmortem ongoing.

No urgent safety recommendation issued yet for Boeing 787-8 or GEnx engines.

🚨 Summary in One Line:
Just after takeoff, both engines on Air India’s Boeing 787-8 VT-ANB unexpectedly shut down due to unexplained fuel switch cutoffs, leading to a total loss of control and a deadly crash into nearby buildings.
