Travel Requirements (Cebu to Siargao)

=Negative swab-based or saliva-based RT-PCR test result
Note: must be 48 hours prior to your flight
I opted for the saliva test at (Red Cross for Php 2,000)
=Confirmed roundtrip tickets
=Confirmed accredited accommodation booking for at least 2 nights
=Valid ID
=Traze app
=Surigao del Norte e-Health Pass
To get your e-HEALTH Pass, sign up in this link:
https://covid19.ssct.edu.ph/healthform
1.Click Register Person
2.Select Tourist as your entry type
3.Fill out the personal information and travel details on the form
4.Click Generate to get your e-HEALTH Pass QR Code.

Submit your complete travel requirements to sdntourism@gmail.com a day before your trip. You will then receive an email if your application to travel is approved.
Requirements:
1.Copy of Negative RT-PCR Swab or Saliva Test Result
2.Copy of a Valid Identification Card or Government-Issued ID
3.Confirmed Roundtrip Airline Tickets
4.Confirmed reservation a a DOT-accredited accommodation
5.Surigao del Norte e-Health Pass
