Your DEMODEX came back after Soolantra? Here’s Why #demodex

What you're experiencing has a name in the literature: the die-off rebound. It's been documented in case reports as recently as 2024. Topical ivermectin kills the adult mites quickly. The eggs - which it doesn't penetrate - hatch on a 14 to 18 day cycle. When the new generation emerges into skin that's already inflamed from the die-off, the symptoms can escalate dramatically. It looks like the treatment failed. It hasn't.

The clinical mistake is stopping. The second mistake is increasing frequency, which intensifies the die-off. The correct move is to extend the protocol to a minimum of 12 weeks, layer in azelaic acid 15% for anti-inflammatory coverage of the hatch cycles, and add nightly lid hygiene with terpinen-4-ol if there's any eye involvement - which there often is when facial Demodex is severe.

Recent case literature also documents ivermectin-refractory cases responding to permethrin 5% combined with low-dose oral isotretinoin. That's a third-line option, but it exists when the standard protocol genuinely fails.
