Dr. Gill discusses the broad topic of the recognition and treatment of adolescent scoliosis. Here are some important takeaways:
-Manipulation has been demonstrated to be ineffective in treating of AIS (Adolescent Idiopathic Scoliosis)
-Team approach to scoliosis
-You must be an expert in this field to understand how scoliosis presents and develops when to treat and what works/does not work
-This scoliosis section “gets your feet wet” but is not an all encompassing treatise to understand scoliosis
-Work closely with a skilled and certified orthotist who designs and fits braces
-You will be responsible to understand when things are not progressing well and when to ask for help
-Must have a relationship with a Spine Surgeon who takes care of AIS

Here is additional information regarding AIS:
-2-4% of children between 10-16 years old
-10 degree or greater curve with vertebral rotation
-Ratio girls/boys; (=) for small curves
-10/1 for curves greater than 30 degrees
-Multi-gene dominant with variable expression
-Normally a right thoracic curve, left lumbar curve
-10% of curves found require intervention

Dr. Gill speaks about the following topics: Natural history of AIS, secondary causes of scoliosis, standing scoliogram x-rays, sagittal balance, coronal balance, secondary causes of scoliosis, normal developmental milestones, Cobb method to measure spine curves, growth remaining indicators, remaining growth skeletal indicators, risk of progression, physical examination, and angle of truncal rotation (ATR) scoliometer.

The goals of treating AIS are:
-Stop curve progression until 2 years post puberty
-Prevent or treat spinal pain syndromes as patients age
-Improve aesthetics (body image)
-Treatment/Referral Guidelines – 25 degrees is a deciding point

Why should a patient have surgery?
-Thoracic curves between 50 and 75 degrees at skeletal maturity progressed an average of 29.4 degrees over a 40.5 year follow-up period
-Curves between 60 and 100 degrees, total lung capacity was 2/3 of predicted normal values
-Half of the patients with thoracic curve larger than 8- degrees had shortness of breath at the average age of 42 years old
-Vital capacity below 45% predicted and a Cobb angle greater than 110 degrees were risk factors to develop respiratory failure and early death
-The larger the curve, the more difficult to correct with surgery
-Body deformity can be cosmetically severe and debilitating

Dr. Gill wraps up his speech speaking about ageing and how larger curves will lead to early degenerative changes, AP & supine bending, surgery, post-surgery, pre and post-op and a handful of cases studies.
