In the 2014 rules, Purple Worms could potentially swallow you instantly on a failed save after landing a bite attack, but the sequence was less structured and more unpredictable. The 2024 edition completely restructures how these underground titans hunt, turning them into brutally efficient killing machines with a specific three-stage attack pattern.

First, the bite attack grapples and restrains you with no save to resist. Second, it uses its multiattack to hit you with the tail stinger for massive poison damage while you're helpless. Finally, it uses a bonus action to swallow you whole, at which point you start taking automatic acid damage at the beginning of its next turn. One round takes you from fighting normally to grappled, poisoned, swallowed, and melting. For players, this creates almost no counterplay window. For DMs, this makes Purple Worms feel like the apex underground predators they should be.
