Slide Deviation Rhinoplasty: Correcting Mild Crooked Noses. Not every crooked nose requires complete reconstruction of the nasal framework.
In selected patients with mild nasal deviation, the nasal pyramid can often be repositioned by working with the high septal strip (upper strip). This technique allows the surgeon to move the entire osseocartilaginous framework—the bony and cartilaginous dorsum—as one stable unit, restoring alignment while preserving as much of the native anatomy as possible.
The concept is based on modifying the upper portion of the septum, which remains connected to the bony dorsum. Once this anatomical unit is carefully mobilized, the entire nasal pyramid can be shifted toward the midline and securely stabilized in its new position.
One of the major advantages of this technique is its usefulness in revision rhinoplasty. Many revision patients have already undergone septal surgery, leaving limited septal cartilage available for reconstruction. Even in these situations, the high-strip concept can provide an effective solution for correcting mild deviations while minimizing additional trauma.
At the same time, the nasal tip cartilages are carefully repositioned and stabilized, creating a balanced relationship between the dorsum and the tip. The goal is not only to achieve a straighter nose but also to preserve natural movement, support, and harmony.
Depending on the patient’s anatomy and the surgical plan, this approach can be performed using either a closed or an open rhinoplasty technique.
Every crooked nose is unique, and no single method is appropriate for every patient. The key is selecting the right technique for the specific pattern of deviation, rather than applying the same solution to every case.
