承煦欲接近國醫，卻驚動侍衛，情急之下躲入茗玉的房間。侍衛闖進，茗玉以清白名譽將其喝退。承煦點破茗玉目的，茗玉也從承煦身上的龍涎香猜出承煦身份。得知承煦此行是為盛州王拿解藥，想到盛州與雍臨利益相關，茗玉決定幫助承煦抓國醫。
茗玉裝病引來國醫，承煦用毒箭相逼，令國醫拿出解藥。國醫使詐，欲讓隨從趁機通風報信，被承煦看穿。
茗玉喬裝成隨從，幫承煦拿到解藥後，國醫著急服下解毒。承煦給國醫強喂五毒散，強迫國醫跟他回盛州，並對外宣稱茗玉臉上的紅疹是染上了時疫。
