BORACAY TRIP REQUIREMENTS:

Step 1: Book your plane ticket (ROUNDTRIP) We suggest that you booked your flight going to Caticlan instead of Kalibo.

Step 2: Book your DOT-accredited hotel accommodation.

Step 3. Book your RT-PCR swab test. (The result must be negative.)

Step 4. Fill-out the Health Declaration Form from https://www.touristboracay.com/

Step 5. Attached all the requirements and e-mail them at touristboracay@gmail.com

-Email subject should be: OHDC, SURNAME, FIRST NAME
-Confirmed Plane Ticket (Roundtrip)
-Confirmed Hotel Accommodation
-Negative RT-PCR Result
-Screenshot of Health Declaration Form
-Proof of Identification with Philippine residency. (1 valid ID per person.)

(You should submit a total of 5 Documents.)
Wait for the requirements to be validated. A copy of the Health Declaration Certificate with the Tourist QR Code will be sent back to you.

Step 6. Download the Traze App.

Step 7. Enjoy!
