In this video, Tommy will teach you how to evaluate a patient with low-flow, low-gradient aortic stenosis. This patient later underwent stress echocardiography. In patients with true aortic stenosis and reduced left ventricular function, the transvalvular gradient will increase whereas the valve area will remain unchanged. This patient's mean gradient increased to over 40 mmHg following dobutamine administration, which was indicative of significant stenosis. He was consequently transferred for surgery and is currently doing well. It can be really tricky to differentiate true- from pseudostenosis.
