"My ankle feels unstable and like it could roll even on completely flat ground."

Here is the neurological reason your ankle keeps rolling: Inside your ankle joint are thousands of tiny nerve sensors called mechanoreceptors and proprioceptors. Their job is to constantly send position signals to your brain so your brain can make micro corrections to keep you balanced. A single ankle sprain tears many of these sensors along with the ligaments. The ligaments often heal but the nerve sensors do not regrow on their own. Your brain is literally getting less information about where your ankle is, so it cannot react fast enough to prevent a roll. The ankle alphabet forces all of those sensors to fire in every direction of movement, which is how you retrain them.

PT exercise: Ankle Alphabet

Step 1: Sit in a chair and lift one foot off the floor so your leg hangs freely. Relax your whole leg and make sure all movement comes from your ankle and foot only, not from your whole leg moving.

Step 2: Using your big toe as a pointer, slowly trace each letter of the alphabet in the air. Make each letter as large and clear as you can. Move slowly and deliberately through A, B, C all the way through Z.

Step 3: Focus especially on any letters that feel stiff or choppy. Those directions are where the proprioceptors need the most work. Go through the full alphabet on both ankles.

Once each ankle daily. Most people notice improved stability and confidence on uneven ground within 3 weeks.
