📍 Cimetidine is an H2 receptor antagonist used for conditions like ulcers, Gastro esophageal reflux disease, and hyper secretory conditions, such as Zollinger Ellison syndrome. this medication works by reversibly inhibiting H2 receptors on gastric parietal cells, which lowers gastric acid secretion.
⚠️ Cimetidine binds to androgen receptors, which can cause impotence, gynecomastia, and lowered libido. It crosses blood-brain barrier, which can lead to confusion and agitation in elderly or severely ill patients, including patients with hepatic or renal impairment. Prolonged acid suppression, can decrease vitamin B12 absorption. If administered rapidly, intravenous cimetidine can cause cardiac arrhythmias.
📍 Cimetidine is an inhibitor of several cytochrome P450 enzymes, so watch out for interactions with medications such as warfarin, beta-blockers, calcium channel blockers, and sulfonylureas. Half life of cimetidine may be increased in patients with renal dysfunction, and dosage adjustments are needed in such patients.
