A 41-year-old lady had platelet-rich plasma treatment on her face, saying about half the usual amount was used, with no hyaluronic acid filler mixed in. She also had a little Botox placed in the corners of her eyes. She developed eye bags since the PRP treatment was performed, so she asks if this is an edema. She says one side of her eye area was bruised during the injection treatment. She read that PRP helps reduce eye bags by increasing blood flow. She was told to have a transconjunctival blepharoplasty, but is asking for a gentler approach. She asks what happened in her case, and for suggestions to resolve this.

New York Oculoplastic surgeon Amiya Prasad, MD reviews her concerns in this video, explaining how PRP cannot cause eye bags, and how eye bag surgery can be done with a more gentle approach:

1:44 - Dr. Prasad’s background training and experience in plastic surgery of the eyes, the Vampire Facelift®, PRP for several different areas, surgical treatment of eye bags, and revision surgery for complications of eye bag surgery from other practices
3:00 - How platelet-rich plasma (PRP) is a concentration of growth and healing factors that can be used to improve skin texture and quality
3:39 - How bruising and swelling is caused by the injection entering the skin, and/or the volume of fluid used in the treatment
3:55 - How swelling can be caused as a response to the stem cell activity and wound healing process that are the basis of collagen stimulation
4:09 - How eye bags are caused by lower eyelid fat prolapse, where the fat behind the skin and the muscle moves forward
4:36 - How lower eyelid fat prolapse can happen with the genetic thinning of the septum, or increased volume of lower eyelid fat
5:15 - How there is an error in cause and effect where two unrelated factors are linked, but PRP itself cannot cause eye bags
5:48 - How the mention of transconjunctival blepharoplasty by a doctor usually means a significant case of fat prolapse
5:55 - How Dr. Prasad developed a gentler approach to transconjunctival blepharoplasty so it is similar in recovery to filler treatment where patients have less bruising even after a week
7:02 - How she can give time to see the degree of her fat prolapse before finding the right doctor to resolve it for her
7:29 - How Dr. Prasad does use PRP in combination with transconjunctival blepharoplasty for his patients, and in lighter skinned patients also uses fractional CO2 laser to rejuvenate the lower eyelid area
