How to treat acne scars- Dermatologist explains
Acne scar treatment is always bespoke, namely for top level Specialist treatment a scar revision program is always designed for the patient. This means not everyone will have the same acne scar treatments. I do not practice the ‘ 4 Fraxel get one Fraxel Free Package’ that many cosmetic practitioners advocate. Specialist, namely Dermatologist and Plastic Surgeons will – or should, find the best methods to treat your acne scar type.

So guys, as much as I would like to help you PLEASE don’t email me and say that you need my help, I can help you, but I need to examine your acne scars in person, namely a traditional face to face examination where I use MY angles of light, watch you animate and also touch your scars to see how tethered or thick scars are. This is the proper way to practice acne scar revision, and that is the only way I do things. After that you may need between one to 12 treatments- once again this depends on your scar type, skin type and downtime. Photos and words are not accurate. Now, here are some acne scar experts I know around the World. If I do not list anyone here, its is because I have not seen their work, but it does not mean they do not exist.

I do not know of anyone in India, Pakistan, Malaysia, or Europe, apart from the UK- Dr Nick Lowe ,Dermatologist, London. Dr Lowe was the pioneer of aesthetic and laser dermatology in Britian.

The first thing to do is to examine under tangential or angled lighting, as this will reveal all. Once this is done, scars can be subtyped and mapped, with high resolution photos. The second aspect, which is equally as important is to examine scars upon animanation, namely when you speak, smile and move your face. This will give me an idea of the amount of tethering and anchored acne scars. Time and time again I get request for ‘what is the treatment’ with static photos, an impossible task to answer correctly. Acne scar assessment has to be done live- with the patient in front of you, and lighting from all angles. Photos do not map scars as well a real time examination under magnification and lighting.

I then select the best method to address our acne scar type. Once again it is the method of acne scar removal that will give the biggest change. Taking note of your skin colour, I devise a plan that may consist of -

Ice pick scars are best treated with chemical peels such as TCA CROSS or punch excision
Rolling scars are best treated with fractional lasers Fraxel, INFINI Microneedling, Dermarollers, skin needling and subscision
Box car scars are best treated with laser resurfacing using a co2 or erbium laser such as Profractional
Atrophic scars are best treated with fillers such as Juvederm, Restylane, Bellafill, or even fat transfer

In this video you will see how Miss Issy got at least 70% of her scars permanent removed using collagen stimulating fillers, HA fillers like Juvederm, low density and spot CO2 full ablation, and most importantly subcision. She had multiple treatments over the year of 2016.

Very rarely do I scars that are uniform- namely lie in the same plane. If I have box car scars and shallow scars in a type one person, one can remove these with fully ablative lasers, including erbium or CO2 laser. One treatment, and they are gone. This procedure can only be done in the minority of scar types, and one has to have fair skin, or skin colour changes will occur.
