CCF - Congestive Cardiac failure

CCF (Congestive Cardiac Failure / Congestive Heart Failure) is a condition in which the heart is unable to pump blood effectively to meet the body's needs. This leads to congestion of blood in the lungs and/or other parts of the body, causing fluid accumulation.
Definition
Congestive Cardiac Failure (CCF) is a clinical syndrome resulting from structural or functional impairment of the heart, leading to inadequate cardiac output and/or elevated filling pressures.
Etiology (Causes)
Cardiac Causes
Coronary artery disease (CAD) / myocardial infarction
Hypertension (high blood pressure)
Valvular heart diseases (mitral or aortic valve disorders)
Cardiomyopathy (dilated, hypertrophic, restrictive)
Congenital heart diseases
Arrhythmias (e.g., atrial fibrillation)
Non-Cardiac Causes
Severe anemia
Thyroid disorders (hyperthyroidism or hypothyroidism)
Chronic kidney disease
Pulmonary hypertension
Excessive alcohol or drug use
Symptoms
Left-Sided Heart Failure
Shortness of breath (dyspnea)
Orthopnea (difficulty breathing when lying flat)
Paroxysmal nocturnal dyspnea (sudden nighttime breathlessness)
Fatigue and weakness
Cough, sometimes with frothy sputum
Right-Sided Heart Failure
Swelling of feet, ankles, and legs (peripheral edema)
Abdominal swelling (ascites)
Enlarged liver (hepatomegaly)
Distended neck veins (raised JVP)
General Symptoms
Reduced exercise tolerance
Weight gain due to fluid retention
Nocturia (frequent urination at night)
Palpitations
Signs
Tachycardia
Tachypnea
Elevated jugular venous pressure (JVP)
Bilateral pitting edema
Basal lung crackles (crepitations)
S3 gallop rhythm
Cyanosis in severe cases
Investigations
ECG
Chest X-ray
Echocardiography (most important)
BNP or NT-proBNP levels
Complete blood count, renal function tests, thyroid function tests
Cardiac biomarkers if myocardial infarction is suspected
Treatment
1. General Measures
Salt restriction
Fluid restriction (in selected patients)
Weight monitoring
Smoking and alcohol cessation
Regular exercise as tolerated
2. Medications
Diuretics (e.g., furosemide) – reduce fluid overload
ACE inhibitors (e.g., enalapril, ramipril) or ARBs
ARNI (sacubitril/valsartan) in suitable patients
Beta-blockers (e.g., carvedilol, metoprolol succinate)
Mineralocorticoid receptor antagonists (spironolactone)
SGLT2 inhibitors (dapagliflozin, empagliflozin)
Digoxin in selected patients
3. Device Therapy
Implantable cardioverter-defibrillator (ICD)
Cardiac resynchronization therapy (CRT)
4. Surgical Treatment
Valve repair/replacement if indicated
Coronary revascularization (angioplasty or CABG)
Heart transplantation in end-stage heart failure
Complications
Pulmonary edema
Cardiogenic shock
Arrhythmias
Thromboembolism
Multi-organ dysfunction
Summary
CCF = Heart's inability to pump blood effectively → reduced cardiac output + fluid congestion.
Common causes: hypertension, coronary artery disease, valvular disease, cardiomyopathy.
Main symptoms: breathlessness, fatigue, leg swelling, orthopnea.
Treatment: lifestyle measures, diuretics, ACE inhibitors/ARNI, beta-blockers, and treatment of the underlying cause.
