Apneustic breathing is a rare but dramatic abnormal respiratory pattern that reveals serious dysfunction within the central nervous system. Normally, the brainstem—specifically the medulla and pons—works seamlessly to regulate the rhythm and depth of breathing. The apneustic and pneumotaxic centers balance inspiration and expiration, ensuring smooth, continuous airflow. When the lower pons is damaged, this balance collapses. The apneustic center becomes overactive, driving a prolonged, gasping inspiration followed by an inadequate, irregular exhalation. As a result, patients appear to take a deep breath, “hold” it, and then release only a partial breath—an alarming pattern that clinicians immediately recognize as apneustic breathing.
This breathing abnormality almost always indicates severe neurological injury. Pontine strokes, traumatic brain injuries, hypoxic events, and brain herniation are the most common causes, though certain drugs or toxins can occasionally produce reversible apneustic-like patterns. Because it reflects significant brainstem dysfunction, apneustic breathing rarely appears alone. It is typically accompanied by impaired consciousness, abnormal reflexes, and other signs of neurological decline. Its presence often signals a poor prognosis.
Patients with apneustic breathing cannot maintain effective ventilation and almost always require mechanical ventilatory support. Respiratory therapists play a crucial role in recognizing the pattern early, alerting the medical team, and initiating appropriate ventilator settings—often in controlled modes that bypass the patient’s ineffective respiratory effort. They also monitor changes in the breathing pattern, which can provide insights into the patient’s evolving neurological status.
Treatment focuses on addressing the underlying cause—such as stabilizing a stroke, relieving intracranial pressure, or reversing hypoxia—while supporting ventilation. Although recovery is possible in rare reversible cases, most instances reflect permanent damage to the pons. Ultimately, apneustic breathing serves as a striking reminder of how deeply breathing is connected to the brain’s most vital functions.
