Adrenal Insufficiency Symptoms and Signs

The presenting signs and symptoms will depend on the following:
1. Rate of loss of adrenal function
2. Depth or the severity
3. Preserved mineralocorticoids or not
4. The degree of the stress
5. Precipitants or not

In Primary Adrenal insufficiency,the symptoms will include:
Shock
Anorexia
Nausea
Vomiting
Abdominal pain
Weakness
Fatigue
Dehydration
Decrease BP
Decrease weight
Hypoglycemia
Fever
Hyperpigmentation
Vitiligo

Central Adrenal insufficiency will have :
Hypothalamic-Pituitary problem=decrease ACTH
Infertility
Amenorrhea
Decreased libido in women
Irregular menses
Hallucinations
Hypothyroidism
Decreased growth
Pituitary apoplexy with headache and decreased v. field etc.

The signs are not completely the same as well e.g.
In Secondary or Tertiary Adrenal Failure:
No hyperpigmentation, because of less ACTH secretion
No dehydration and less hypotension
No hyperkalemia
Less GIT symptoms
More Hypoglycemia in secondary
Other hormone deficiencies
