Due to the extreme timescales, a few flags from the medieval period could be inaccurate, as well as a few territorial evolutions.

The flag of the Almohad dynasty is wrong.
The Teutonic Order should've been conquered in 1237.
Kakatiya, Pandya kingdoms had a flag before 1279.
Cilicia should've ended in 1375.
Wrong Abyssinia flag from 1019 -1542.
Wrong Kievan Rus flag.
The Hausa shouldn't have any flag.
The Lordship of Ireland should appear in 1171.
Wrong medieval Japanese flag before the Akishaga Shogunate.
The appearance of the Mayan flag could be inaccurate.
Srivijaya Empire should fall in 1377.
Majapahit should appear in 1293.
The flag of Oman before the 17th century shouldn't be shown.
The Karamanids should appear in 1250.
The Sultanate of Rum should fall in 1308.
The Medieval Serbian flag could be inaccurate.

Ava Kingdom was should be shown in 1364.
Malayapura and Malacca had a flag before 1443.
Malwa wasn't part of the Delhi Sultanate from 1414 - 1451.
Kalmar Union should have existed during 1457 - 1464, 1497 - 1501.
Tondo Kingdom (in the Philippines) might have had a red flag.
The Kingdom of Cyprus should've fallen in 1448.
The Butua Kingdom should be shown from 1019 onward.
Possible inaccurate evolutions of the flags of the Holy Roman Empire and Byzantine Empire (Before 1300.)
Spain claimed the Canaries in 1483.
Portugal should have Ceylon, (1640 - 1661.)
Luanda Kingdom should have split up before 1665.
Sweden should not have occupied the Polish Lithuanian commonwealth after 1679.
Crete should have belonged to the Ottomans after 1670.
Cyprus should have belonged to the Ottomans after 1571.
Sweden should have had the St. Petersburg area before the war of the Spanish Succession.
The Dutch should have had Mauritius and Reunion between 1638 and 1715.
The Arcot State should have been first shown in 1690, not 1737.
The UK occupied the mouth of the St Lawrence river for to long. (1730s - 1750s)
Various parts of the French Empire claimed in 1790 should have been claimed earlier.
Portugal should have had a colony in modern Guinea - Bissau in 1785, not 1814.
Jolof should have disappeared in 1549, not 1830.
Madeira became Portuguese in 1425, not 1860.
Bukhara should've changed it's flag in 1785, not 1868.
Sakhalin should have been entirely under Russian control in 1875, not 1892.
Various parts of the British Empire within the South Atlantic should have been claimed before 1900.
