We begin by defining paraneoplastic syndromes as immune-mediated or hormonally driven phenomena caused by tumor-derived antibodies, cytokines, or hormone-like substances. The lecture categorizes them into endocrine, neurologic, hematologic, and dermatologic subtypes—emphasizing their underlying mechanisms and the malignancies most commonly associated with each.
The session then examines classic examples, such as SIADH and Cushing syndrome associated with small cell lung carcinoma, Lambert-Eaton myasthenic syndrome caused by anti-voltage-gated calcium channel antibodies, and paraneoplastic cerebellar degeneration linked to anti-Yo and anti-Hu antibodies. We also review hematologic manifestations, such as migratory thrombophlebitis in pancreatic cancer, and dermatologic findings, including acanthosis nigricans and dermatomyositis.
Finally, we highlight the diagnostic approach, which includes antibody testing and imaging to locate occult tumors, followed by management strategies that focus on treating the underlying malignancy and modulating the immune response through the use of corticosteroids, IVIG, or plasmapheresis.
