Case 73: PCI Manual: LIMA PCI

A patient with prior CABG and ischemic cardiomyopathy was referred for diagnostic angiography and PCI. He has an abdominal aortic aneurysm hindering equipment advancement. Using a JR4 diagnostic catheter and a Glidewire the aneurysm was successfully crossed allowing performance of coronary angiography.
A decision was made to intervene to a distal anastomotic lesion of the LIMA. Attempts to insert an Impella via femoral access failed due to a severely calcified lesion in the distal aorta. Given bilateral IMA grafts axillary Impella insertion was not considered a good option. PCI of the LIMA anastomosis was performed with nice final result without hemodynamic compromise.
