The open radical hysterectomy is done for early stages of invasive cancer cervix.
In 1898, Ernst Wertheim, a Viennese physician, developed the radical total hysterectomy with removal of the pelvic lymph nodes and the parametrium. In 1905, he reported the outcomes of his first 270 patients.Meigs repopularized the surgical approach when he developed a modified Wertheim operation with removal of all pelvic nodes.In 1901, Schauta described the radical vaginal hysterectomy and reported a lower operative mortality rate than the abdominal approach. In the late 20th century, radiation therapy became the favored approach .esp for advanced cancer.In 1921, H. Okabayashi published his own technique, developed with his professor, S. Takayama, which had as particularity, the preservation of the nerve plexus After a few years, Navratil (1950) and Subodh Mitra (1951) improved the vaginal hysterectomy method by adding the extraperitoneal ilio-obturator lymphadenectomy.
At present, there are three classifications of radical hysterectomies which are used for the simplification of the surgical protocols: Piver-Rutledge-Smith classification which is the oldest, GCG-EORTC( GCG-EORTC = Gynecologic Cancer Group of the European Organization of Research) classification and Querlow and Morrow classification. The last is the most evolved and recent classification; its techniques can be adapted for conservative operations and for different types of surgical approaches: abdominal, vaginal, laparoscopic or robotic.
