Inquiry into killings by veteran calls for more medical file sharing

The inquiry that investigated why former soldier Lionel Desmond killed three family members and himself in 2017 has released 25 recommendations on how to prevent a similar tragedy — including more case workers for veterans and sharing of medical files.
