Pocket Masks and Oxygen administration

The pocket mask is an effective way of being able to deliver breaths into somebody and forming a barrier between you and the patient. Now, this is a typical case that the pocket mask comes in. It has a small clip on the end, you can open that up and expose the pocket mask. What you have got with the pocket mask is, it crushes so it fits in the case. All you need to do is push your thumbs on the end and push it out, and now the mask is ready for use. The elastic strap could be then be used to place the mask over the patient's face and goes behind the head. So it keeps it in place. So this is used when we are doing resuscitation. Because it's nice and firmly around the back of their head, we don't need to worry about placement, because it will sit there quite firmly.

What we have got on the mask is an air-filled sack around the outside. So as it is on there, it will seal very well around the face. What we can then do is take our hands and seal around the mask to ensure it is held in place, while opening the airway. And then we can deliver the breaths in through the mouthpiece. The way this mouthpiece works is, it's a one-way valve. So the air's going in through here and then it's exhaled here. So you are not getting someone else's breath coming back at you. So if the patient's got blood in their mouth, if they've got vomit, you are keeping yourself away from that. It's a much more pleasant way of delivering rescue breaths.

These units themselves are disposable. Inside here is a one-way valve. You can remove this valve and take that off and throw it away and put another one on. But imagine if you did have to use one of these for real, it's likely you can have vomit on the elastic. And it's something that you would just throw away and start again. They are not really designed to be re-used on that side. If you are using them within training, yeah you can clean them, if you are doing exercise you can clean them and re-use them again.

Now, the other feature of a pocket mask is you can give supplementary oxygen. Now, you would only give supplementary oxygen if you are trained to do so. So we are just going to just briefly go through how oxygen is delivered within the pocket mask. But remember, you would not do this unless you have training. On here is a little outlet, so what we have got is a cap which you can just remove. You can take your standard oxygen tubing and push that onto the outlet. So now we have got a constant feed of oxygen going into the mask. Turn the oxygen tank on, preset the flow rate and you set that to 15 litres a minute. And then you would blow in here. And what's happening then is the air that you are blowing in to inflate their lungs is being mixed with the pure oxygen going in here. And rather than resuscitating them with 16% oxygen, remember we breathe in 21%, we breathe out 16%, your body uses around about five. we are breathing 16% in here, mixed with 100% here, so roughly speaking we can increase this oxygen percentage to somewhere around about 50%, which is much better. If we are doing resuscitation, we can put a much higher concentration of oxygen in, which would give the person a much better chance of possibly being able to be resuscitated.
