With a pilot chute in tow and the ground approaching quickly, James pulls his cutaway followed by his reserve, but the RSL gets snagged, causing an entanglement on opening.

Some of you may point the finger at the RSL as the sole cause of this incident — historically and statistically, RSLs have saved far more lives than caused issues. Others might point the finger at emergency procedures — some of us are taught to go straight for the reserve with a pilot chute in tow instead of cutaway/reserve.
