Accidental IV Injection of LA

In this video we share one of NYSORA’s Compendium’s techniques to decrease the risk of an accidental IV injection of local anesthetic. Such an injection during regional anesthesia can result in severe LA systemic toxicity. Ultrasound reduces but does not eliminate the risk of intravascular injections. During the performance of nerve blocks, the operator if often busy monitoring the ultrasound screen, the patient, and the vital signs, forgetting to check the tip of the needle hub for blood, while this is exactly where the first indication of an intravascular needle placement can be detected. At the same time, the helper person expects to see the blood in the syringe, which actually rarely happens. Unless the helping person observes the injection tubing at the needle hub, they can easily miss an intravascular needle placement. The outlines the technique to decrease the chance of missing an IV injection.
