Mentalization-based treatment (MBT) is one of the major three evidence-based treatments for borderline personality disorder (BPD). Recent elaboration of this model has indicated a new understanding of the core problem in BPD as impaired epistemic trust—that is, difficulty trusting sources of learning about the world, including one’s own mind and the minds of others. Dr. Unruh summarizes a mentalization approach to BPD built upon this new understanding of why people with BPD can be “hard to reach,” even by loved ones and expert clinicians.
