How to intubate? Mannequin (OSCE)

The video is done to mainly demonstrate the intubation, for the airway assessment please read it separately, the video is an aid to study,
Sequence to intubation,:
1- consent, hand hygiene and gloves

2- Airways assessment:

- Flexion and extension of neck
- Mallampati classification
- "3-2-1 rule" Š thyromental distance, distance of lower mandible in midline from the mentum to the thyroid notch; less than 3 finger breadths less than 6 cm, is associated with difficult intubation Š

- mouth opening less than 2m finger breadths is associated with difficult intubation anterior jaw subluxation less than 1 finger breadth is associated with difficult intubation.

4- intubation:
- ensure equipment is available and working, test ETT cuff, check laryngoscope light
- Proper position, sniffing position
- pre-oxygenate/denitrogenate: patient breathes 100% O 2 for 3-5 min or for 4 vital capacity breaths
- Intubate
- Secure tube with tapes after procedure is done

5- confirm proper intubation:

- Symmetrical chest movement
- Auscultation with symmetrical air entry in both lungs
- ET in exhaled gas measured by capnograph CO2
- CXR (rarely done)
