1) Take the dog/puppy to a certified veterinarian for “health certificate” or some vets call them “travel certificate”. The vet will perform a physical exam to determine if the pet is healthy enough to travel. The health certificate can be no more than 10 days old from the date of travel

2) Gather the name, address and phone number of both shipper and receiver (consignee) as it appears on both IDs

3) Call United Pet Safe at (800) 575-3335. A live person will help you book the flight. Make sure to note the drop off and pick up locations at both airports. Some airports drop off at the ticket counter, others have an off site cargo department address. Also note your journey number and airway bill number

4) At least 5 days prior to flight, submit the following 4 items to United’s online portal at:

Scanned copy of the completed and signed PetSafe Customer Acknowledgement Form
www.unitedcargo.com
Scanned copy of your pet's Health Certificate from a licensed veterinarian dated within 10 days of travel or as required by the destination country
Digital photo of your pet clearly showing their face to verify breed
Digital photo of your pet's travel crate to verify compliance with PetSafe's crate requirements
Use your journey number on all correspondence

5) Buy an airline approved crate. Must have hard sides and open air ventilation on all 4 sides, plus must be screwed securely together on all 4 corners. Your puppy must be able to stand, sit and turn around without touching his ears or tail to the top or sides of the crate. For shipping small animals less than 5 pounds, For small dogs under 5 pounds I use the 19” Pet Taxi from Walmart.

6) Buy 2 Prevue Birdie Basics Perch Cups off Amazon or at your local feed store. Make sure to note the size of the container to be sure it fits inside your crate. Two 4oz bowls will fit into the 19” pet taxi

7) For several days leading up to the flight, let your dog our puppy spend some time in the crate. Put their favorite blanket inside and prop the door open for him to come and go as he pleases. By flight day, we want him to be comfortable inside the crate

8) Add several handfuls of shredded newspaper to absorb any potty accidents!

9) Leave the water and food bowl empty but attach them inside the crate. Bring the following with you to the drop off location:

-Valid ID matching the name on the reservation
-Puppy inside crate with absorbent material and 2 empty dishes (must be attached to the inside of the crate)
-Zip Lock baggie of the puppy's food (they will tape it to the top of the crate for you at the airport)
-Two copies of the signed and completed Customer Acknowledgement Form
-Print two copies of a photo of your pet, clearly showing your pet’s face (image should be no smaller than 3” x 3” and no larger than 4” x 6”)
-Health Certificate completed and signed by a licensed veterinarian no more than 10 days prior
-Any additional required documents for travel to the destination (for international travel, required documents should be confirmed by your IPATA shipper)

10) Arrive at the cargo drop off location at least 2 hours prior to flight time. You will need to show ID and all documentation and fill out some forms and make payment at the time of drop off. Most airports accept card only - no cash.

11) Make sure the consignee will be waiting at the pick up location at the time of arrival. They must have a Valid ID matching the information on the reservation for pet to be released
