This is Past Squid game reacts, the characters aren't mine but deigns and reactions are in gacha.

The old man is smiling because he sees that everything worked out how he wanted it to and he got to play the games one last time. Also because of his disease he's disorientated, just like in the series he's not really "there" (mentally) most of the time (Green light red light: he's smiling because he doesn't really pay attention to the deaths but rather on having fun) he's not meant to be evil or to be smiling at people's deaths. He's just happy his plan for the game went smoothly and he could die knowing he has fun.
