NCCN Guidelines for Ampullary Adenocarcinoma – Diagnosis and Management
This educational video provides a concise overview of the National Comprehensive Cancer Network (NCCN) Guidelines for the management of ampullary adenocarcinoma, a rare gastrointestinal malignancy arising from the ampulla of Vater. Ampullary cancers account for a very small proportion of GI tumors but require multidisciplinary management involving surgical oncology, medical oncology, gastroenterology, and radiation oncology.

The video explains the diagnostic evaluation, staging, and treatment strategies recommended by NCCN. Key topics include the role of imaging and endoscopy, histological classification (intestinal vs pancreatobiliary subtype), and treatment decision-making based on disease stage. The standard treatment for localized resectable disease is pancreaticoduodenectomy (Whipple procedure) with the goal of achieving an R0 resection.

The discussion also covers adjuvant systemic therapy, which is frequently recommended after surgical resection to reduce the risk of recurrence.

n addition, the video reviews the management of locally advanced, unresectable, and metastatic disease, where systemic chemotherapy becomes the primary treatment modality. Radiation therapy or chemoradiation may be considered in selected cases, although strong evidence for its benefit remains limited.

This lecture is designed for surgical oncologists, gastroenterologists, oncology trainees, and medical students, aiming to simplify the NCCN treatment algorithms and provide a practical understanding of current evidence-based management strategies for ampullary adenocarcinoma.
