Gout - Singapore Podiatrist Michael Lai, DPM

Gout is a common disease in Singapore. It can affect almost anyone; the elderly, younger adults, athletes and in some cases, children with obesity, diabetes or kidney disease.

People suffering from Gout experience excruciating pain at the affected area, even covering the foot with a blanket could be too painful.

Active gout flare-ups with intense swelling and redness would require oral medications dispensed by your doctor or rheumatologist. With a history of gout, your podiatrist will be able to help you to prevent long term joint damage.

Gout results from hyperuricemia. That is, from too much uric acid in the bloodstream. A doctor would order a blood test if he suspects that your joint pain is from gout. Attacks are typically acute onset, can be recurrent, and initially monoarticular (affecting only 1 joint).

The big toe joint is involved in 90% of people with gout. That is why podiatrists become quite experienced in dealing with this arthritic condition.

Uric acid crystals accumulating within a joint is what triggers gouty attacks. A rheumatologist can manage the gout with proper medications.

However, many patients don’t realise that gout is not just crystals causing temporary inflammation; gout can also permanently affect the bones, and result in joint deterioration and destruction. Imagine an apple being eaten from the inside out by worms. The apple is your bone and joints; the worms is gout-induced erosion.

If joint damage has already taken place, then medications alone will not resolve the problem. There may even be knock-on effects of a joint compromised by gout, causing secondary musculoskeletal pains in other areas of the foot and ankle. Podiatrists can address the resulting joint pain and deformity by adjusting the foot biomechanics, using shoe modifications and custom orthotic insoles for long-term relief.
