VATS converted to VAMT (Video assisted Mini Thoracotomy) radical excision of mediastinal tumor.

@drprriya Minimally edited video of VATS converted to VAMT - Video assisted mini thoracotomy radical excision of mediastinal tumour with wedge resection of lung in a Primary mediastinal NSGCT post chemotherapy with BEP Regime. Excellent response to chemotherapy but its share of dense adhesions and desmoplasia. Dense adhesions to pericardium and pulmonary artery - led to conversion to mini thoracotomy. When at the top - due to adhesions with PA - it was a touch and go. Either it would be a R2 or there was high chance of injury. A haptic feedback helped tide over and the surgery went smooth. Recovery uneventful. All margins free. No viable tumour. No further chemotherapy as advised by the medical oncologist. To be kept under follow up.
Lessons learnt :
1. Do no underestimate the power of BEP - good or bad both !
2. Patience is the key
3. At the end of the day - R0 and Safety both matter.
4. Haptic feedback is a very important tool in some situations. Especially like these. Take some time. and if need arises - open a bit.
